6. Discharge
By the end of the three-month program, our clinicians review each participant’s progress against their individual action plan to determine whether ongoing support is needed or if they are ready to transition out of the program.
5. Ongoing Support
Participants will engage in either in-person or telehealth sessions, scheduled weekly or fortnightly depending on their individual needs and goals. These regular check-ins provide consistent, structured support, allowing clinicians to monitor progress, address challenges as they arise, and adjust strategies to ensure participants are moving steadily toward their agreed outcomes.
In partnership with each participant, we develop customised plans with clear, achievable, and measurable goals tailored to their individual needs.

2. Assessment
Comprehensive evaluation of each participant’s values, strengths, and personal priorities is undertaken to ensure support is meaningful, person-centred, and aligned with what matters most to them. This process helps identify existing capabilities, motivations, and protective factors that can be built upon, forming a strong foundation for goal setting and ongoing support.
3. Collaborative Planning
Our qualified clinicians work closely with each participant to develop a tailored action plan that reflects their individual goals, needs, and circumstances. This process is strengths-based and participant-led, ensuring goals are realistic, meaningful, and achievable. The action plan is designed as a practical roadmap, outlining clear steps, supports, and strategies to guide progress throughout the program.
4. Regular Updates
We work collaboratively with referring and support providers throughout the program to ensure coordinated care and continuity of support. Regular progress updates are provided to relevant providers, outlining each participant’s engagement, achievements, and any emerging needs. This ongoing communication helps ensure all parties remain informed and aligned in supporting the participant’s goals throughout the duration of the program.
Our ongoing follow-up system ensures participants stay on track, with strategies adapted as circumstances change.

Established in 2024 by Dan Dennis, Mind Ready was built from a genuine commitment to helping people gain (and maintain) employment, build confidence, and regain direction in their lives.
Dan brings over a decade of frontline social work, clinical practice and sector education to his role as Managing Director of Mind Ready Support Services. His career spans key positions across NSW Communities and Justice as a Caseworker, and NSW Health as a VAN Social Worker, JCPRP Health Clinician and Clinical Coordinator.
Dan’s experience supporting individuals and families through complex challenges — including trauma, safety concerns and mental health barriers — has shaped Mind Ready’s strengths-based and holistic approach. He is committed to empowering clients through personalised assessment, coordinated support and ongoing guidance that leads to long-term, sustainable change.
Under Dan’s leadership, Mind Ready continues to champion dignity, resilience and the belief that every person has the capacity to thrive when provided with the right tools, support and opportunities.
